Patient PDF Forms - Troubleshooting

If you are experiencing problems viewing the ARAPC Patient PDF Forms,
please read the following instructions:


Option 1:

Make sure your Adobe Reader software is up to date

Adobe releases regular updates for Reader and Acrobat. It's likely that the latest update patch can fix a conflict with your system.

1) Open Reader or Acrobat.
2) Choose
Help > Check for Updates.
3) Exit Adobe Reader
4) Re-open Adobe Reader and open the ARAPC PDF Form


Option 2:

Don't have Adobe Reader on your computer?
Download Adobe Reader on your computer. The software is FREE.

If your using a
Windows computer, CLICK HERE

Make sure to "uncheck" the additional offer in the middle of the page, then click "Install Now"

If your using a Mac (Apple) computer, CLICK HERE
Follow Steps 1, 2 & 3 and click "Download Now"
